What is an Incident?

An incident is any event that causes or could cause harm to a participant, staff member, or others. This includes:

  • Injuries or medical emergencies
  • Abuse, neglect, or exploitation
  • Medication errors
  • Property damage or loss
  • Breaches of privacy or confidentiality
  • Complaints or allegations of misconduct

Our Approach

  1. Prevention: Identify and minimize risks through training, policies, and safe practices
  2. Response: Take immediate action to ensure safety and provide support
  3. Reporting: Document and report incidents as required by law and NDIS requirements
  4. Investigation: Investigate thoroughly to understand what happened and why
  5. Learning: Implement improvements to prevent similar incidents

Reporting an Incident

Emergency: Call 000

For immediate danger, serious injury, or medical emergency

For non-emergency incidents, contact:

  • Your support coordinator or manager immediately
  • Update coordinator or AussieLife Care on 1300 000 000

NDIS Reportable Incidents

We are required to report certain serious incidents to the NDIS Quality and Safeguards Commission within 24 hours, including:

  • Death of a participant
  • Serious injury or illness
  • Abuse or neglect
  • Unlawful sexual or physical contact
  • Unauthorized use of restrictive practices
