What is an Incident?
An incident is any event that causes or could cause harm to a participant, staff member, or others. This includes:
- Injuries or medical emergencies
- Abuse, neglect, or exploitation
- Medication errors
- Property damage or loss
- Breaches of privacy or confidentiality
- Complaints or allegations of misconduct
Our Approach
- Prevention: Identify and minimize risks through training, policies, and safe practices
- Response: Take immediate action to ensure safety and provide support
- Reporting: Document and report incidents as required by law and NDIS requirements
- Investigation: Investigate thoroughly to understand what happened and why
- Learning: Implement improvements to prevent similar incidents
Reporting an Incident
Emergency: Call 000
For immediate danger, serious injury, or medical emergency
For non-emergency incidents, contact:
- Your support coordinator or manager immediately

NDIS Reportable Incidents
We are required to report certain serious incidents to the NDIS Quality and Safeguards Commission within 24 hours, including:
- Death of a participant
- Serious injury or illness
- Abuse or neglect
- Unlawful sexual or physical contact
- Unauthorized use of restrictive practices
